Job summary

Qualifications

• Two years' experience as a Registered Nurse in a clinical care setting preferred.
• Nursing skills as defined by generally accepted standards of practice.
• Reliable transportation required.

Responsibilities

• Provide nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary group and patient/family.
• Attend IDG meetings and provide supervisory visits.
