Job summary

Frontpoint Health is seeking a PRN Hospice Registered Nurse (RN) in Eden, TX to provide nursing care at patients' homes. You will coordinate with the interdisciplinary group, patients, families, and referring agencies while managing plan-of-care tasks.

Requirements include Texas RN licensure (NCL accepted), 2 years clinical RN experience (hospice/home health preferred), CPR and TB screening per policy, and reliable transportation. A $2,500 sign-on bonus is offered.

Qualifications

• Licensed RN in Texas or NCL-compliant licensure.
• Two years of RN experience in clinical care; hospice/home health preferred.
• Strong communication and interpersonal skills; sensitivity to loss and grief.
• Reliable transportation and valid state driver’s license.
• CPR certification and TB test per policy, Hepatitis B vaccination.

Responsibilities

• Promote hospice mission and provide care per Plan of Care.
• Perform information visits, eligibility assessments and admissions.
• Assess patient/caregiver needs and educate as indicated.
• Coordinate care with IDG, patient, family and physician; attend IDG meetings.
• Provide supervisory visits for hospice aides as applicable.
• Document services and submit timely; participate in on-call duties.
