Job summary

Frontpoint Health is seeking a PRN Registered Nurse (RN) to administer nursing care in patients' homes in Fort Worth, TX. The role coordinates with the Interdisciplinary Group and families to ensure quality, continuous care at the end of life.

The RN will perform initial and ongoing assessments, establish levels of care, implement plans of care, and educate patients and families on disease processes, self-care, and nutrition.

Qualifications

• Graduate from an accredited Diploma/Associate/Baccalaureate School of Nursing.
• Current state RN license or compliance with Nurse Licensure Compact rules.
• Two years RN experience in a clinical care setting; hospice experience preferred.
• Read, write and comprehend English.

Responsibilities

• Assess patients' physical, functional, psychosocial, and environmental needs.
• Coordinate care with IDG/Team, patients, families, and referring agency.
• Develop and document plan of care; revise as needed to ensure quality and continuity.
• Educate patients and families on disease, end-of-life care, nutrition, and self-care; train staff as needed.
• Assess acuity to determine scope and frequency of services.
• Provide infection control and standard precautions in patient care.
• Direct Hospice Aide and LPN/LVN to ensure quality of service.
• Document progress notes and communicate changes to physicians and team.
• Monitor cases for third-party payor requirements.
• Perform on-call duties and provide on-call services.
