Job summary

Elara Caring is seeking a dedicated Registered Nurse in Alabama to deliver high-quality home health care. You will perform comprehensive patient assessments, develop and update plans of care, and work closely with physicians and interdisciplinary teams to ensure compassionate, goal-directed care in patients’ homes.

Candidates should have an active RN license, 1+ year of nursing experience, and reliable transportation. Hospice/home health experience is preferred.

Qualifications

• State license as a Registered Nurse is required.
• 1+ year nursing experience in a clinical setting.
• Ability to lift, push, pull, and transfer patients up to 50 pounds.
• Must have valid driver's license or reliable transportation to travel between patient homes.
• Hospice or home health experience is preferred.

Responsibilities

• Assess patients’ physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.
• Complete initial evaluations and follow-up reassessments.
• Monitor patient condition and promptly communicate changes to the physician and care team.
• Update the Plan of Care as needs change and coordinate revisions with the care team.
• Educate patients, families, and facility staff on the Plan of Care and hospice philosophy.
• Collaborate with Medical Director, attending physician, branch leadership, and interdisciplinary team to coordinate care.

Education

Graduate of an accredited nursing program (Certificate/Diploma/Associate/Baccalaureate)

Responsibilities

• Educate and support patients, families, caregivers, and facility staff on the Plan of Care, hospice philosophy, and pain and symptom management.
• Collaborate with the Medical Director, attending physician, branch leadership, facility staff, and interdisciplinary care team to coordinate patient care.

Requirements

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to handle multiple priorities, documentation requirements, and deadlines.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.
