Job summary

Elara Caring is hiring a Registered Nurse to join our home health team in Oklahoma. You will assess patients, develop and update the Plan of Care, and work with physicians and a multidisciplinary team to ensure timely, compassionate care.

The role requires an active RN license, 1+ year of clinical nursing experience, ability to lift up to 50 pounds, and reliable transportation for travel to patient homes. Hospice/home health experience is preferred.

• Must possess a valid driver's license or reliable transportation for travel between patient homes.

Responsibilities

• Assess patients' physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.
• Complete initial evaluations and reassess nursing needs during follow-up visits.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Update the Plan of Care as patient needs change and coordinate revisions.
• Educate patients, families, caregivers, and facility staff on the Plan of Care and pain management.
• Collaborate with Medical Director, attending physician, and interdisciplinary team to coordinate care.

As a Registered Nurse, you'll contribute to our success in the following ways:

• Assess patients’ physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.

• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Update the Plan of Care as patient needs change and coordinate revisions with the appropriate care team members.
• Educate and support patients, families, caregivers, and facility staff on the Plan of Care, hospice philosophy, and pain and symptom management.
• Collaborate with the Medical Director, attending physician, branch leadership, facility staff, and interdisciplinary care team to coordinate patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to handle multiple priorities, documentation requirements, and deadlines
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
