Job summary

Elara Caring is seeking a Registered Nurse Home Health to provide high-quality nursing care to patients in their homes across Terre Haute, IN and surrounding areas. You will assess patient needs, implement plans, educate families, and coordinate with the care team to ensure safe, effective care.

Ideal candidates hold an active RN license, have at least 1 year of clinical nursing experience, and can travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate or Bachelor).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds; travel between patient homes.

Job Description

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
