Job summary

Interim HealthCare Hospice is seeking a compassionate PRN Hospice RN to provide home-based care to patients and families during after-hours and weekends. You will collaborate with a physician, LPN, CNA, HHA and other team members to implement the care plan and ensure comfort and quality of life.

The role requires an RN license, CPR certification, and the ability to work independently with strong communication and critical thinking skills.

Qualifications

• RN license is required.
• CPR certification required.
• Valid car insurance.
• Hospice experience 1+ year preferred.
• Strong critical thinking and problem-solving skills.
• Excellent communication and interpersonal skills.
• Proficiency in medical terminology.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide home-based hospice care after-hours, weekdays and weekends as needed.
• Help patients spend quality time with loved ones and participate in their care.
• Collaborate with an interdisciplinary team to carry out the plan of care.
• Perform patient assessments, monitor vitals, breathing, pain; administer prescribed meds and treatments.
• Ensure access to equipment, oxygen, medications and supplies.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do

• Provide home-based care to hospice patients after-hours, weekdays, weekends, and as needs arise
• Enable patients to spend quality time with loved ones and participating
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family.

A few must-haves for our Hospice Registered Nurses

• Registered Nurse (RN) license
• CPR certification
• Valid car insurance
• Prior Hospice experience 1+ years
• Strong critical thinking and problem-solving skills
• Excellent communication and interpersonal skills
• Proficiency in medical terminology and understanding of Hospice nursing care
• Ability to work independently and manage time effectively.
• Triaging any emergent needs and collaborating with physicians to provide comfort and quality care to each patient
• Communicating with medical professionals, the hospice team and others
• Maintaining accurate, up-to-date records
• Providing services in accordance with agency policies and regulations that define scope of practice
