Job summary

Interim HealthCare Hospice is seeking a Hospice Registered Nurse in Michigan to provide compassionate, home-based care to patients with limited life expectancy. You will collaborate with an interdisciplinary team to implement the patient’s plan of care, assess and monitor symptoms, and educate families on prognosis and hospice goals.

The role emphasizes autonomy, flexible assignments, ongoing online training, and opportunities for CEUs, with a 4:30pm-8:00am Monday-Thursday schedule.

Qualifications

• Active, unrestricted RN license in Michigan.
• Minimum 1 year of RN experience.
• Home health or hospice experience preferred.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide home-based care to patients with six months or less to live.
• Work with interdisciplinary team to carry out patient’s plan.
• Perform patient assessments, vitals, breathing, pain, medications and treatments.
• Educate family on prognosis and care plan.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Take a holistic approach to care engaging the patient’s mind, body, spirit and family.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Hospice Registered Nurses:

• RN with active, unrestricted license in the State of Michigan
• At least one (1) year of relevant experience as an RN (required)
• Community health and a wide range of Medical-Surgical experience (preferred)
• Reliable transportation with a valid state driver\'s license and current car insurance
