Job summary

Interim HealthCare Hospice is seeking a Hospice Registered Nurse to provide compassionate, home-based care for patients with limited life expectancy.

You will work with an interdisciplinary team to carry out each patient’s plan of care and ensure comfort and quality of life in the home setting.

Qualifications

• RN license active and unrestricted in Michigan is required.
• Minimum of 1 year of RN experience.
• Community health exposure and broad Medical-Surgical experience preferred.

Responsibilities

• Provide home-based care to patients with six months or less to live, focusing on quality of life.
• Collaborate with an interdisciplinary team (physician, LPN, CNA, HHA, social worker, chaplain, volunteers) to implement patient plans of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, and administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, oxygen, medications, and other supplies.
• Educate families on prognosis, care plan changes, medications, and hospice goals.
• Take a holistic approach to care involving patient, family, mind, body, and spirit.

Schedule: Monday through Thursday (4:30pm-8:00am)

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

A few must-haves for Hospice Registered Nurses:

• RN with active, unrestricted license in the State of Michigan
• At least one (1) year of relevant experience as an RN (required)
• Community health and a wide range of Medical-Surgical experience (preferred)
• Reliable transportation with a valid state driver's license and current car insurance
