Job summary

Interim HealthCare Hospice is seeking a compassionate PRN Hospice RN to provide high-quality, home-based nursing care for patients at the end of life. You will be the face of courage and the supportive arms for patients and families, helping them find comfort and closure during this significant season.

Qualifications

• RN license required.
• CPR certification required.
• Valid car insurance/driver's license.
• Hospice experience 1+ year preferred.
• Strong communication and interpersonal skills.
• Proven time-management and independent work ability.
• Proficiency in medical terminology and hospice care basics.

Responsibilities

• Provide home-based hospice nursing care after-hours, weekdays, weekends as needed.
• Support patients to spend meaningful time with loved ones and comfort them.
• Collaborate with an interdisciplinary team to implement the plan of care.
• Perform patient assessments, monitor vitals, manage symptoms and medications.
• Ensure accessibility of equipment, oxygen, medications and supplies.
• Educate families on prognosis, care plan changes and hospice goals.
• Deliver holistic care addressing mind, body, and family needs.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do

• Provide home-based care to hospice patients after-hours, weekdays, weekends, and as needs arise
• Enable patients to spend quality time with loved ones and participating
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family.

A few must-haves for our Hospice Registered Nurses

• Registered Nurse (RN) license
• CPR certification
• Valid car insurance
• Prior Hospice experience 1+ years
• Strong critical thinking and problem-solving skills
• Excellent communication and interpersonal skills
• Proficiency in medical terminology and understanding of Hospice nursing care
• Ability to work independently and manage time effectively.
• Triaging any emergent needs and collaborating with physicians to provide comfort and quality care to each patient
• Communicating with medical professionals, the hospice team and others
• Maintaining accurate, up-to-date records
• Providing services in accordance with agency policies and regulations that define scope of practice
