Job summary

Interim HealthCare Hospice is seeking a Hospice Registered Nurse to provide compassionate, home-based care for patients with limited life expectancy. You will collaborate with an interdisciplinary team to implement care plans, monitor patient status, and educate families on hospice goals.

Responsibilities

• Provide home-based care to patients with six months or less to live, focusing on quality of life.
• Work with an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medications and supplies.
• Educate family on prognosis, care plan changes, medications and hospice goals.
• Take a holistic approach to care engaging the patient and family.

Schedule: Monday through Thursday (4:30pm-8:00am)

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Hospice Registered Nurses:

• RN with active, unrestricted license in the State of Michigan
• At least one (1) year of relevant experience as an RN (required)
• Community health and a wide range of Medical-Surgical experience (preferred)
• Reliable transportation with a valid state driver\'s license and current car insurance
