Health Link Home Health and Hospice in San Francisco seeks compassionate Hospice Registered Nurses (RNs) for per diem or part-time roles in the field. You will provide skilled nursing in patients\' homes and residential settings, focusing on comfort, dignity, and quality of life.

You will collaborate with an interdisciplinary team and educate patients and caregivers on medications and symptom management, traveling across San Francisco as needed.

Formación

• Current and unrestricted RN license in California.
• Hospice or home health experience is highly preferred.
• Strong clinical assessment, critical-thinking, communication, and organizational skills.
• Comfortable managing patients independently in the field while collaborating with an interdisciplinary team.

Responsabilidades

• Provide skilled nursing care in accordance with each patient’s hospice plan of care.
• Perform comprehensive nursing assessments and ongoing evaluations of patients’ needs.
• Provide pain and symptom management to promote comfort and quality of life.
• Administer medications and treatments and evaluate patient response.
• Monitor changes in patient condition and communicate with the hospice team.
• Educate patients, families, and caregivers about medications and end-of-life care.
• Coordinate care with physicians, hospice aides, social workers, chaplains, and facilities.
• Complete accurate and timely clinical documentation using Kinnser/WellSky.
