Job summary

Health Link Home Health & Hospice is seeking an experienced Registered Nurse (RN) to join our Transitions Department in San Francisco. The Transitions Nurse collaborates with Home Health teams to assess changing patient needs and coordinate hospice, palliative care, or other community resources, ensuring timely, appropriate care.

Qualifications

Current and unrestricted California RN license required.
1 year nursing experience preferred in Home Health, Hospice, or transitions.
Strong communication and patient-family education skills.

Responsibilities

Identify changing patient needs and coordinate appropriate services or levels of care.
Proactively contact patients and families to discuss options and provide education.
Coordinate transitions to hospice, palliative care, or higher levels of care.
Maintain ongoing communication and follow-up with patients and families.
Document interactions, care plans, and transition status accurately.
