Job summary

HealthFlex Home Health Services is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to provide skilled nursing care and coordinate end-of-life services for patients and families in the Bay Area.

Schedule options include Full Time, Part Time, or Per Diem. The role involves travel to the Oakland office for weekly IDG meetings and collaboration with an interdisciplinary team to ensure quality patient care.

Qualifications

• CA license required as RN in good standing.
• Hospice case management experience required (1+ year).
• Current CPR certification (AHA).
• Valid driver’s license and ability to travel to Oakland office for weekly IDG meetings.
• Self-directed and able to work with minimal supervision.

Responsibilities

• Conduct initial and ongoing patient/family assessments to identify hospice needs.
• Provide nursing care using the nursing process.
• Develop and initiate a patient care plan with family involvement.
• Re-evaluate patient needs and revise plans as necessary.
• Coordinate care with physician and interdisciplinary team.
