American Medical Home Health & Hospice in San Antonio, TX is seeking a compassionate Hospice Registered Nurse (RN) for PRN/Per Visit work. You will provide skilled nursing and end-of-life care to patients in homes and community settings, collaborating with an interdisciplinary team and utilizing WellSky EMR.

You will conduct admissions, assessments, and routine visits, focusing on comfort, symptom management, and patient-family education.

Qualifications

• Current and valid Texas RN license
• Minimum of one year nursing experience preferred
• Hospice experience strongly preferred
• Home health, palliative care, oncology, geriatric, or acute care experience is a plus
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently in the field
• Reliable transportation, valid driver's license, and current auto insurance
• Ability to travel throughout the San Antonio service area
• Professional, compassionate, and patient-focused approach to care

Responsabilités

• Complete hospice admissions, assessments, and routine nursing visits
• Assess patient needs and identify changes in condition
• Develop, implement, and update individualized plans of care
• Manage pain and symptoms to maximize patient comfort and quality of life
• Administer medications, treatments, and therapies as ordered
• Educate patients, families, and caregivers regarding disease progression, symptom management, and end-of-life care
• Coordinate care with physicians, social workers, chaplains, hospice aides, facility staff, and other interdisciplinary team members
• Communicate significant patient changes promptly to appropriate team members
• Complete documentation accurately and within required timeframes
• Ensure compliance with hospice regulations, agency policies, and quality standards
