Job summary

Health Link Home Health and Hospice is seeking compassionate Hospice Registered Nurses (RNs) for San Francisco on a per diem or part-time basis. You will care for patients at home or in residential settings, coordinating with an interdisciplinary team to deliver comfort, dignity, and quality of life.

Responsibilities include assessments, pain and symptom management, education for patients and caregivers, and timely documentation using Kinnser/WellSky. Travel within San Francisco is required.

Qualifications

• Current and unrestricted RN license in California.
• Minimum of 1 year of nursing experience.
• Hospice or home health experience preferred.
• Strong clinical assessment, critical-thinking, communication, and organizational skills.
• Comfortable managing patients independently in the field while collaborating with an interdisciplinary team.
• Ability to travel throughout San Francisco.

Responsibilities

• Provide skilled nursing care per the patient’s hospice plan of care.
• Conduct comprehensive nursing assessments and ongoing evaluations.
• Manage pain and symptoms to promote comfort and quality of life.
• Administer medications and treatments and evaluate patient response.
• Monitor patient condition and communicate with the Hospice Clinical Director and team.
• Educate patients, families, and caregivers on medications and care plans.
• Provide emotional support to patients and families during hospice journey.
• Coordinate care with physicians, hospice aides, social workers, chaplains, and facilities.
• Participate in interdisciplinary team meetings and care planning.
• Complete accurate documentation using Kinnser/WellSky on company devices.
• Maintain compliance with regulations and safety requirements.
• Serve as a clinical resource and advocate for patients.
