Job summary

Hospice of the East Bay in Pleasant Hill, CA is seeking a Per Diem Registered Nurse to provide direct nursing care and coordinate services for patients and families in the home setting. This role requires an active California RN license and CPR certification, with a preference for hospice or home care experience.

Qualifications

• Current California Registered Nurse License required.
• Minimum of one year of experience as a professional nurse in an acute clinical setting.
• CPR certification required.
• Must have a valid California Driver’s License and a car in good working condition.

Responsibilities

• Provide direct nursing care and coordinate related services for patients and families.
• Perform complete nursing physical assessments and develop comprehensive care plans.
• Document problems, assessments, goals, care provided and outcomes.
• Update patient progress at least every 15 days and complete documentation within 2 hours of shifts.
• Collaborate with interdisciplinary team; participate in team meetings and hand-off communications.
• Deliver patient and caregiver education and training as needed.
• Adhere to state, federal regulations and accreditation standards.
