Job summary

Journey Hospice is seeking a per diem RN Case Manager to provide skilled nursing care to hospice patients in West Caldwell, New Jersey. This flexible role allows for working as needed and offers competitive pay of up to $47 per hour.

The RN will manage a caseload, perform comprehensive assessments, collaborate with the care team, and ensure patient needs are met in accordance with legal and agency standards.

Qualifications

• Graduated from an accredited registered nursing program.
• NJ RN licensure in good standing.
• Minimum one year general nursing experience.

Job description

Journey Hospice offers a rewarding per diem RN Case Manager position. This role is flexible, offers up to $47/hour, and allows you to work as needed. The RN will oversee a primary caseload of hospice patients and provide skilled professional nursing care to patients, families and caregivers as prescribed by the physician and in compliance with Medicare Conditions of Participation, New Jersey hospice licensure laws, accrediting standards, and agency policies and procedures.

Responsibilities include: initial assessment, comprehensive nursing assessment, identification of problems, reassessment, collaborate with interdisciplinary team to develop care plan, implement and revise care plan, teach and counsel patients, families, and staff, supervise hospice aides/homemakers, and work closely with team members to meet patient and family needs.

Education / Training / Certifications

• Graduated from an accredited registered nursing program.
• BSN preferred.
• Certified Hospice and Palliative Nurse (CHPN) preferred.
• NJ RN licensure in good standing.
• Valid driver’s license and reliable insured transportation in accordance with NJ state requirements.

Experience

• Minimum one year general nursing experience with current hospice, home health, medical, surgical or critical care experience.
• Experience with pain and symptom management interventions preferred.
• Ability to work within an interdisciplinary setting.
• Understanding of hospice philosophy and principles of death and dying.
