Per Diem Nurse Practitioner – Hospice

We are seeking a Per Diem Nurse Practitioner to work with our hospice patients in Essex County. The position is per diem with no guaranteed hours. Nurses are paid $125 per client visit. The role requires working under the general supervision of the Hospice Medical Director and performing patient medical examinations, palliative care visits, documentation in the clinical record, and communication with the interdisciplinary team.

Responsibilities

• Conduct patient medical examinations and palliative care visits as required for hospice care.
• Document visits in the patient’s clinical record and required forms.
• Provide medical care services and palliative care intervention for hospice patients on an as‑needed basis.
• Communicate effectively with the Medical Director, Administrator, and the Hospice Interdisciplinary Team.

Qualifications

• Education / Training / Certifications:
• Graduation from a master’s‑level program designed to educate and prepare nurse practitioners/clinical nurse specialists.
• Completion of a graduate‑level 3‑credit course in Pharmacology from a nationally accredited agency or a pharmacology course of at least 45 hours integrated into the master’s program.
• National certification requiring a successfully passed examination.
• Licensing and Experience:
• Currently licensed and nationally certified via the New Jersey Board of Nursing as an Advance Practice Nurse in New Jersey (N.J.A.C. 13:37-7.1).
• Minimum of two (2) years’ experience as a Nurse Practitioner.
• Valid driver’s license and reliable transportation insured in accordance with New Jersey requirements.
• Skills:
• Provide services in a timely and professional manner in accordance with hospice bylaws and operational policies.
• Demonstrate a positive, professional manner and manage time efficiently with a team‑approach.
• Develop and maintain effective relationships with the interdisciplinary group, other professionals, and physicians.
• Maintain a professional attitude and demeanor; communicate effectively and collaborate cooperatively with patients, caregivers, colleagues, supervisors, and others.
• Show compassion toward patients in a hospice care setting and reflect concern for the wellbeing of patients and employees.
