Job summary

Lutheran Social Ministries of New Jersey is seeking a Per diem RN Case Manager to oversee the care of Hospice patients. In this role, you will provide skilled nursing care, collaborate with an interdisciplinary team, and ensure that patient needs are met.

Candidates should possess a nursing degree, NJ RN Licensure, and preferably have CHPN certification. This position offers flexible scheduling on a per diem basis, allowing you to balance work with personal commitments.

Job description

Per diem RN Case Manager

Pay rate: up to $47 per hour.

The Hospice RN will oversee primary caseload of Hospice patients and provide skilled professional nursing care to them, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures. The RN provides the initial assessment, comprehensive nursing assessment, and identification of problems for each patient; reassesses nursing needs, and collaborates to develop the Hospice Interdisciplinary Team (IDT) plan of care. The RN implements the plan of care, reevaluates and revises the plan relative to the patient’s nursing needs to provide appropriate, comprehensive, and responsive goals and interventions. The RN teaches, supervises, and counsels the patient, family members, and staff regarding nursing care and the patient's needs, including related problems of the patient at home. The RN works closely with other members of the IDT to ensure all patient and family care needs are met and provides supervision to Hospice Aides/Homemakers.

Education / Training / Certifications

• Graduate of an accredited registered nursing program.
• Bachelor’s degree in Nursing preferred.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred.
• NJ RN Licensure by the NJ State Board of Nursing in good standing.
• Valid driver’s license and reliable transportation that is insured in accordance with NJ state requirements.

• Understanding of Hospice philosophy, principles of death/dying.
