New Jersey, Inc. is seeking a Per Diem RN Case Manager to oversee hospice patients and provide skilled nursing care. This role includes assessing patient needs, developing care plans, and collaborating with the Hospice Interdisciplinary Team.
Qualifications include being a graduate of an accredited nursing program with a current NJ RN Licensure, and a minimum of one year of nursing experience is preferred. The position offers flexible scheduling at a competitive hourly rate of up to $47.
Per diem, as needed, with flexible scheduling and pay rate up to $47 per hour.
The Hospice RN will oversee a primary caseload of hospice patients and provide skilled professional nursing care to them, their families and caregivers in accordance with physician orders and compliance with the Hospice Medicare Conditions of Participation, New Jersey Hospice licensure laws, accrediting body standards, and agency policies and procedures. Responsibilities include providing the initial and comprehensive nursing assessments, identifying patient problems, reassessing nursing needs, and collaborating to develop the Hospice Interdisciplinary Team (IDT) plan of care. The RN implements, reevaluates, and revises the plan of care to meet the patient's nursing needs, delivering appropriate, comprehensive and responsive goals and interventions. The RN teaches, supervises, and counsels patients, family members, and staff regarding nursing care and the patient's needs, including related problems at home. The RN works closely with other IDT members to ensure all patient and family care needs are met and provides supervision to Hospice Aides/Homemakers.
