Job summary

Moments Hospice is seeking a dedicated on-call RN to cover nights and weekends on a full-time schedule (7 days on/7 days off). The role involves after-hours assessments, care planning, and coordination with families in patients' homes and communities.

The position requires RN licensure in the state of work, EMR proficiency, and strong planning, organization, and communication skills. A company car, health benefits, and PTO are part of the package.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Perform initial and ongoing patient assessments for hospice needs during after-hours calls.
• Respond to triage calls, make home visits, and handle weekend/night shifts.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family involvement.
• Administer medications and treatments per physician's plan.
• Educate patients and families on self-care and care options.
• Maintain accurate clinical notes and document changes.
