AMEDISYS HOME HEALTH is hiring for a Hospice Weeknight On Call RN. Respond to after-hours telephone requests from our triage RN, providing skilled assessment and interventions, including possible after-hours admissions and end-of-life visits.

Schedule is Monday to Thursday, 5pm to 8am, four on-call nights. Salary ranges from $83,200 to $86,000 with mileage; benefits begin the first month and a comprehensive suite of health and wellness programs are offered.

Qualifications

• One+ year of nursing experience required.
• Current RN license, state-specific.
• Current CPR certification required.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes and revises the total plan of care with continuity across the care team.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families.

Respond to telephone requests at home from our after-hours Triage RN for assistance by addressing the request by telephone or by making an in person visit that includes skilled assessment and interventions which can include in person after hours admissions and end of life visits

Attractive pay

• $83,200 - $86,000 Salaried + mileage
• Benefits first of month after hire
• Schedule - Monday - Thursday 5pm - 8am - (4 nights a week on call)
• (2nd on call weeknight position) Wilmington or North of Wilmington would be ideal

• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One+ year of nursing experience required
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
