Job summary

Amedisys, Inc. seeks a Hospice Weeknight On Call RN to respond to after-hours requests from the home setting, offering skilled assessment and interventions by telephone or in-person visits that may include after-hours admissions and end-of-life visits.

The role is salaried with mileage, featuring a Monday–Thursday 5pm–8am schedule (4 nights on call) with opportunities for relocation to Wilmington area or nearby north of Wilmington as described.

Qualifications

• Current RN license in the state(s) you will work.
• Minimum 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Overview

Hospice Weeknight On Call

Respond to telephone requests at home from our after-hours Triage RN for assistance by addressing the request by telephone or by making an in person visit that includes skilled assessment and interventions which can include in person after hours admissions and end of life visits

Attractive pay

• $83,200 - $86,000 Salaried + mileage
• Benefits first of month after hire
• Schedule - Monday - Thursday 5pm - 8am - (4 nights a week on call)
• (2nd on call weeknight position) Wilmington or North of Wilmington would be ideal
