Job summary

Amedisys is seeking an on-call registered nurse for Monday-Thursday (5pm-8am) shifts in West Columbia, SC. The role involves initial assessments, coordinating care plans, and communicating patient progress to the care team, while maintaining accurate records and educating families.

Qualified candidates hold an active RN license for the relevant state(s) and CPR certification, with at least one year of nursing experience.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Coordinates and revises the total plan of care with staff.
• Communicates patient progress to the clinical manager and care team.
• Maintains individualized patient care records with accuracy and timeliness.
• Records pain/symptom changes and manages outcomes.
• Educates families on patient condition and care needs.
• Performs on-call responsibilities and other duties as assigned.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
