Job summary

Amedisys is seeking a Hospice Weekend On Call Baylor RN to respond to after-hours requests from our triage team. The role involves assessing patients at home, deciding on telephone guidance or in-person visits, and coordinating with the care team to ensure timely, compassionate care.

You will work weekends, with salary between $84,000 and $86,000 per year, plus mileage reimbursement. The position includes comprehensive benefits from the first of the month and opportunities to support patients

Qualifications

• Current RN license for the state(s) assigned.
• One+ year of nursing experience.
• Current CPR certification.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates and revises the total plan of care with the care team.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Develops, prepares, and maintains patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.

Hospice Weekend On Call Baylor RN

Respond to telephone requests at home from our after-hours Triage RN for assistance by addressing the request by telephone or by making an in person visit that includes skilled assessment and interventions which can include in person after hours admissions and end of life visits

Attractive pay

• $84,000 - $86,000 Salaried + mileage

• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• One+ year of nursing experience required
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
