Job summary
Ascend Hospice & Palliative Care MA is seeking an RN On Call Nurse (Weekdays) to provide on-call hospice care across a defined territory. You will assess, plan, implement, and evaluate nursing care while coordinating with physicians, other hospice staff, and agencies as needed.
The role requires an Associate's Degree in Nursing and a state RN license, with hospice or home health experience, strong documentation, and excellent communication skills. Travel within the assigned territory is expected.
Qualifications
- State Certified RN license required.
- Experience in hospice or home health care required.
- Excellent documentation skills and ability to work independently.
- Strong communication skills are essential.
- Valid driver's license and reliable transportation; willingness to travel locally.
Responsibilities
- Assumes responsibility for patient visits or on-call schedule across nursing process phases.
- Provides hospice nursing care focused on pain control and symptom management.
- Communicates with attending physicians and the care team to coordinate patient care.
