Qualifications

- Must be available to travel locally within assigned territory.
Responsibilities
- Assesses, plans, implements and evaluates nursing care for patients on call.
- Provides hospice care focused on pain control and symptom management.
- Coordinates with physicians, hospice staff and agencies to plan care.
- Maintains up-to-date documentation and accurate charges.
- Communicates changes in patient condition to the IDT and supervisor.
- Participates in on-call duties and ensures equipment and services are arranged.
