Job summary

Ascend Hospice & Palliative Care MA in Worcester, MA is hiring an RN On Call Nurse for weekend coverage. The role includes patient visits, on-call scheduling, pain and symptom management, and coordinating with physicians and the care team.

Requires Associate’s Degree in Nursing, State RN license, hospice or home health experience, strong documentation and communication skills, valid driver’s license, and ability to travel within the territory. Competitive salary and benefits offered.

Qualifications

• Associate’s Degree in Nursing required.
• State Certified RN license required.
• Hospice or home health care experience preferred.
• Excellent documentation skills and ability to work independently.
• Strong communication skills and compassionate patient care.
• Valid driver’s license and reliable transportation.
• Must be able to travel locally within assigned territory.

Responsibilities

• Assesses, plans, implements and evaluates the nursing process for the patient.
• Provides hospice nursing care focused on pain control and symptom management.
• Initiates communication with attending physicians and other care teams to coordinate care.
• Maintains up-to-date documentation per agency policy and Medicare regulations.
• Administers medications in accordance with state and federal laws.
• Communicates with the Medical Director to determine patient eligibility and care plans.
• Participates in on-call duties as defined by policy.
• Ensures equipment and service arrangements are in place as needed.
