Job summary

St. Croix Hospice in Newburgh, IN is seeking an experienced On-Call Registered Nurse to organize and direct hospice care in home and community settings. This full-time role covers nights and weekends, requiring nurse licensure in Indiana and CPR certification.

You will assess patients, develop and revise care plans, administer medications, counsel families, and coordinate with physicians while delivering compassionate, patient-centered care during after-hours calls.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Current state licensure and CPR certification.
• 1–2 years acute care experience; 1–2 years home care experience.

Responsibilities

• Respond to after-hours calls and visit patients with changing conditions.
• Perform initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Initiate and revise plans of care based on patient status and needs.
• Administer medications and treatments as prescribed by the physician.
• Counsel patients and families in nursing and related needs.
• Coordinate with physician and community health team; educate patient and family.
• Maintain accurate clinical notes and documentation.
