Job summary

St. Croix Hospice is seeking an On-Call Registered Nurse to organize and direct hospice care in patients' homes and communities. The role emphasizes after-hours calls, night and weekend coverage, and coordination with the interdisciplinary team to meet patient needs.

The ideal candidate will have an active RN license, CPR certification, and experience in home care, with strong assessment and communication skills to support patients and families during challenging times.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state and CPR certification.
• Experience in home care and acute care settings preferred.

Responsibilities

• Respond to calls from Triage during assigned shifts.
• Perform comprehensive patient and family assessments for after-hours calls.
• Provide professional nursing care using the nursing process.
• Develop and update plan of care with patient and family involvement.
• Administer medications and treatments as prescribed.
• Educate patients and families on self-care and management.
