Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse On-Call to join our 24/7 care model. You will respond to triage calls and provide after-hours visits for symptom management, admissions, and care plan updates, while developing individualized care plans for evolving patient needs.

You will administer medications, educate patients and caregivers, and maintain precise clinical documentation. This role offers autonomy, work-life balance, and is centered on delivering dignified

Qualifications

• Graduate of an accredited nursing program (Associates, Bachelors, or Masters).
• 1+ year of recent acute care experience; hospice or home care preferred.
• Current RN license in state of practice and CPR certification.

Responsibilities

• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and treatments as prescribed, with patient and caregiver education.
• Maintain timely and accurate clinical documentation of care, outcomes, and communication.
