Job summary

St-Croix Hospice is seeking an On-Call Registered Nurse to organize and direct hospice care in patients' homes and communities. The role covers nights and weekends, with after-hours triage, assessments, and plan-of-care development.

You will administer medications, counsel patients and families, and coordinate with an interdisciplinary team to provide comprehensive, patient-centered care. Strong nursing and communication skills are essential.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting, and one to two years in home care.
• Current licensure in State and CPR certification.

Responsibilities

• Triage and respond to after-hours calls, visiting patients with changing conditions.
• Perform comprehensive assessments of patients and families to determine hospice needs.
• Develop and revise plans of care; initiate appropriate nursing procedures and medications.
