Job summary
Interim HealthCare Hospice is seeking a Hospice RN to provide compassionate home-based care to patients with limited life expectancy in Michigan. You will assess needs, administer prescribed meds, and support families within an interprofessional team.

Qualifications
- RN with active, unrestricted license in Michigan.
- At least one year of RN experience required.
- Community health and medical-surgical experience preferred.
- Reliable transportation with valid driver's license and current insurance.
Responsibilities
- Provide home-based care to patients with six months or less to live.
- Work with an interdisciplinary team to carry out the patient’s plan of care.
- Perform patient assessments, check vitals, monitor pain and treatments.
- Ensure accessibility of medical equipment, oxygen, meds and supplies.
- Educate family on prognosis, care plan changes and hospice goals.
