Job summary

Suncrest Hospice is seeking a Resource Nurse (RN) to deliver high-quality hospice care in home settings across the service area. You will visit patients in private homes, SNFs, and assisted living facilities as part of an interdisciplinary team.

Shift is Monday–Friday 3p–11p, with negotiable timing. Ideal candidates hold an active RN license, excel in teamwork, and manage patient visits, documentation, and meetings efficiently.

Qualifications

• Current RN licensure in good standing in practice state(s).
• Ability to collaborate with an interdisciplinary care team.
• Capable of independent work and effective time management.

Responsibilities

• Provide in-home visits and care to a group of patients within service area.
• Contribute clinical expertise on psychosocial assessments and care planning.
• Coordinate with nurses, social workers, chaplains, and CNA leadership.
