Job summary

Gentiva seeks an experienced Hospice Clinical Liaison to manage intake, referrals, and admissions. You will build strong professional relationships with hospitals, physicians, case managers, and facilities to coordinate timely admissions and ensure quality patient care.

You will educate families about hospice options, review medical records, and supervise documentation to meet regulatory and payer requirements while supporting community awareness of hospice services.

Qualifications

• Licensed nurse with 3–5 years hospice experience required.
• BSN with 5+ years hospice experience preferred.
• Minimum of 3 years hospice experience total.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, and other healthcare professionals.
• Assess patient eligibility for hospice services by reviewing records and communicating with physicians/families.
• Educate patients and families on hospice services and how they work.
• Coordinate intake and admission, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and the hospice team for seamless handoffs.
• Make daily visits to hospitals and facilities to stay connected with discharge planners and physicians.
• Build relationships with referral partners and identify outreach opportunities.
