Job summary

Gentiva Health Services, Inc. is seeking an experienced Hospice Clinical Liaison to ensure efficient intake and referrals, coordinate admissions, and support community awareness of hospice services. You will build strong professional relationships and liaise with discharge planners, physicians, and families.

Ideal candidates hold an RN license, CPR, and 3+ years of hospice experience; BSN is preferred. This role requires compassionate patient advocacy and the ability to navigate complex payer

Qualifications

• Active RN license in the state of employment.
• CPR certification.
• BSN preferred with hospice experience.
• Minimum of 3 years of hospice experience.

Responsibilities

• Receive and respond to referrals from hospitals, home health, physicians, case managers, SNFs, and other professionals.
• Assess patient eligibility for hospice services via medical records review, clinical evaluations, and physician/family communication.
• Educate patients and families about hospice care, processes, and benefits with clarity and compassion.
• Complete intake and admission paperwork, ensuring legal, clinical, and payer requirements are met.
• Serve as liaison between referral sources and hospice care team, ensuring smooth handoffs and timely communication.
• Make daily visits to hospitals and facilities to stay connected with discharge planners and physicians.
• Build and maintain relationships with referral partners and seek new outreach opportunities.
