Job summary

The Pennant Group, Inc. is seeking a Hospice Registered Nurse to deliver high-quality care in patients’ homes within the North Phoenix and Scottsdale area. You will act as a trusted case manager, guiding families through care decisions with compassion and clinical excellence.

The role emphasizes comprehensive assessments, individualized plans, medication management, and effective communication with physicians and the care team. A Monday–Friday schedule and travel are involved.

Qualifications

• Currently licensed as an RN in Arizona.
• Minimum of 2 years of nursing experience; hospice or home health preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical skills and patient-centered care.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans, revising as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities appropriately.
