Job summary

Suncrest Hospice is seeking an experienced Registered Nurse (RN) to provide care for a patient group, visiting residents in homes, SNFs, and assisted living facilities. You will be an integral part of the interdisciplinary team, offering psychosocial assessments and linking patients to community resources.

The role supports Case Management, Admissions, or Resource functions and requires strong teamwork, independence, and excellent time management while meeting hospice regulations and ensuring

Qualifications

• Current RN license in good standing in applicable states.
• Team-oriented with daily collaboration with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Patient-focused, providing 1:1 care with a broader care team support.
• Ability to work independently and manage communications via phone, email, and meetings.
• Strong time management to schedule visits and meetings and triage urgent needs.
• Willingness to drive across the service area with reliable transportation.

Responsibilities

• Provide care and services to a group of patients in homes and facilities.
• Visit patients where they reside (private homes, SNFs, assisted living).
• Contribute to the interdisciplinary team with psychosocial assessments and interventions.
• Utilize community resources knowledge to assist patient care.
• Work in Case Management, Admissions, or Resource roles.
