Job summary

Suncrest Hospice is seeking a Registered Nurse to provide care for a group of patients in home, SNF, and assisted living settings. You will be part of an interdisciplinary team, delivering psych-social assessments and coordinating with community resources.

This role covers Fremont, San Jose, or Peninsula areas; schedule Wednesday-Saturday 8:30 AM–7 PM; requires active RN licensure in the state; must collaborate with nurses, social workers, chaplains, volunteers and CNA leadership; valid driver's
