Job summary

Suncrest Hospice is seeking an Registered Nurse (RN) to provide in-home and facility-based care across the service area. You will join an interdisciplinary team to deliver compassionate hospice services and perform psych-social assessments, coordinating with social workers and other professionals.

The role requires current RN licensure and willingness to travel. Expect collaborative work with a strong emphasis on patient-centered care, documentation in EMR systems, and opportunities for growth

Qualifications

• Current RN licensure in state(s) of practice.
• Works well within a team environment and collaborates with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Able to work independently with calls, emails, and meetings while following hospice rules.
• Willingness to travel across the service area with reliable transportation.

Responsibilities

• Provide care and services for patients in homes or facilities visited by the RN.
• Be part of an interdisciplinary team, providing psych-social assessments and resource knowledge.
• Serve in Case Management, Admissions, or Resource roles as part of care coordination.
• Maintain documentation and communication to support patient care plans.
