Job summary

Suncrest Hospice is seeking a Registered Nurse to provide compassionate, individualized hospice care in patients' homes, skilled nursing facilities, and assisted living communities. You will collaborate with an interdisciplinary team of nurses, social workers, chaplains, CNAs, and volunteers to develop and implement care plans that address physical, emotional, and spiritual needs of patients and families.

Qualifications

• Current RN license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary care teams.
• Ability to work independently while effectively managing your daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and utilizing electronic medical record (EMR) systems.
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care wherever patients call home—from private residences to SNFs and assisted living communities.
• Collaborate with an interdisciplinary team to develop and implement comprehensive care plans.
