Suncrest Hospice in the United States seeks a dedicated Registered Nurse to provide compassionate care to patients in homes and facilities across the service area.

You will join an interdisciplinary team, contributing clinical assessments, care planning, and coordination across Case Management, Admissions, or Resource roles. Travel required with reliable transportation; licensure in good standing.

Qualifications

• Current Registered Nurse licensure in good standing.
• Works well in a multidisciplinary team with nurses, social workers and chaplains.
• Capable of independent daily visits and clear documentation.
• Strong time management and prioritization for multiple visits and meetings.
• Familiar with hospice rules and community resources.

Responsibilities

• Provide care to patients in homes or facilities as part of a multidisciplinary team.
• Work in Case Management, Admissions, or Resource roles.
• Coordinate visits and communications with other healthcare professionals.
