Top Dog Recruiting seeks an experienced Hospice RN Clinical Liaison to build referral relationships with Bedford and Manchester-area hospitals. This hospital-focused role covers the NH Greater Manchester area, not bedside nursing.
You will educate clinical teams and families about hospice services, manage referrals, eligibility discussions and admission workflows, and drive census growth through proactive outreach.
Qualificações
- Active, unencumbered RN license.
- Hospital bedside nursing background (med-surg).
- Minimum 1 year of healthcare sales or marketing experience.
- Ability to build relationships with hospital clinical teams and physicians.
Responsabilidades
- Develop and maintain relationships with hospital referral sources and healthcare professionals.
- Makes regular calls to secure new referrals and meet goals with physicians, facilities, case managers, discharge planners and social workers in hospitals and skilled nursing facilities.
- Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
- Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
- Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
