Job summary

KOB Solutions, Inc. is seeking a Hospice RN Clinical Liaison / RN Hospital Liaison in Greater Manchester, NH. This full-time role covers Bedford and Manchester-area facilities and focuses on hospital referral relationships rather than bedside care.

You will develop relationships with hospital teams, conduct regular outreach to secure referrals, educate on hospice services, and support admission discussions to drive census growth.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside background, including med-surg.
• Minimum 1 year healthcare sales or marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license, auto insurance and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular calls to secure new referrals and grow census with physicians, facilities, and case managers.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, eligibility discussions and admission/consent processes as appropriate.
• Drive hospital referral activity and census growth through service-focused outreach.
