Job summary

Clear Concepts, Inc. is seeking a skilled Hospice RN Clinical Liaison to serve hospital referrals in the Greater Manchester area. This is a full-time position (Mon–Fri, 8:00 AM–4:30 PM) focused on building hospital partnerships rather than bedside nursing.

You will cultivate relationships with hospital teams, discharge planners and physicians, educate about hospice services, and support eligibility discussions and admissions to grow referrals and census.

Qualifications

• Active, unencumbered RN license.
• Hospital bedside nursing background (med-surg).
• Minimum 1 year healthcare sales/marketing experience.
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
• Valid driver’s license and dependable transportation.

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Makes regular, planned calls to secure new referrals and meet established goals with physicians, facilities, case managers and discharge planners in hospitals and SNFs.
• Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
