Meritas Advisory Group, LLC in Greater Manchester, NH is seeking an RN Hospice Clinical Liaison who partners with hospitals to grow referrals. This full-time, Monday–Friday role focuses on Bedford and Manchester-area facilities rather than bedside nursing.

The RN will educate clinical teams and families about hospice services, facilitate admissions, and drive census growth through proactive outreach to physicians, discharge planners, case managers and social workers in hospitals and SNFs.

Qualifications

• Active RN license
• Hospital bedside nursing background including med-surg experience
• Minimum 1 year of healthcare sales or marketing experience
• Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians
• Valid driver’s license, auto insurance and dependable transportation

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Makes regular, planned calls to secure new referrals, build census, and meet established goals on new and existing referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
• Educate patients, families and clinical teams about hospice services and appropriate transitions of care.
• Support referrals, patient eligibility discussions and the admission/consent process as appropriate.
• Drive hospital referral activity and hospice census growth through consistent, service-focused outreach.
