Job summary

Connectly Recruiting in Manchester, NH is seeking an RN Hospice Liaison to develop and maintain relationships with hospital referral sources and healthcare professionals. This full-time, Monday–Friday role covers Bedford and Manchester-area facilities, focusing on education of patients and clinical teams about hospice services, transitions of care, and enrollment processes.

Ideal candidates bring hospital bedside nursing experience, strong communication skills, and a drive to grow hospice census

Qualifications

• Active RN license in NH
• Hospice or hospital liaison experience preferred
• Ability to build relationships with hospital teams
• Valid driver’s license and reliable transportation
• 1 year healthcare sales or marketing experience

Responsibilities

• Develop and maintain relationships with hospital referral sources and healthcare professionals.
• Make regular calls to secure referrals, build census, and meet goals with physicians, facilities, case managers, and discharge planners.
• Educate patients, families and clinical teams about hospice services and transitions of care.
• Support referrals, patient eligibility discussions and admissions processes.
• Drive hospital referral activity and census growth through service-focused outreach.
