Job summary
Job Juncture is confidentially seeking a hospice liaison RN for hospital-based referrals in the Manchester, NH area. This is not a traditional bedside nursing role but a hospital-focused position requiring clinical strength, referral development, and the ability to build strong relationships with hospital teams.
The role covers Bedford and Manchester-area facilities, with relocation not available. A valid RN license and hospital bedside experience are essential, along with healthcare sales
Qualifications
- Active, unencumbered RN license.
- Hospital bedside nursing background, including med-surg experience.
- Minimum 1 year of healthcare sales or marketing experience.
- Ability to build relationships with hospital clinical teams, discharge planners, case managers and physicians.
Responsibilities
- Develop and maintain relationships with hospital referral sources and healthcare professionals.
- Make regular calls to secure new referrals and meet goals with physicians, facilities, case managers and social workers in hospitals and SNFs.
- Educate patients, families and clinical teams about hospice services and transitions of care.
- Support referrals, patient eligibility discussions and the admission/process as appropriate.
- Drive hospital referral activity and hospice census growth through service-focused outreach.
