Graham Healthcare Group in Moosic, PA is seeking a Registered Nurse (RN) Hospice to provide end-of-life care to patients in their homes. This Staff position offers a $10,000 first-year bonus and a 40-hour workweek with 8-hour shifts.

You will collaborate with an interdisciplinary team, perform patient assessments, manage symptoms, educate families, and maintain accurate EMR documentation while traveling within the assigned territory.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license and ability to travel ~20% of each day.

Responsibilities

• Deliver compassionate, patient-centered hospice care at home.
• Perform comprehensive assessments and manage symptoms.
• Educate patients and families about end‑of‑life care.
• Ensure compliant EMR documentation and timely communication.
• Travel to patient homes within assigned territory.
